PARENTAL CONSENT FORM

Parents must agree to all items and this signed form must be submitted to Hands of Hope America R.I.S.E Summer Camp. Please keep a copy of this page and read it carefully before signing.


I understand that the R.I.S.E Summer Camp is being conducted by Hands of Hope America, Inc., and that while attending the R.I.S.E Summer Camp, my child will participate in programs and activities offered by Hands of Hope America.


I understand that accidents and injuries may occur during participation in such activities, and that every reasonable effort will be made to provide reasonable care by the camp staff.


I understand that my child may participate in field trips offered by Hands of Hope America Summer Camp, including field trips to the pool with transportation provided by a Van. I understand the risks involved in attending a swimming pool.


I hereby give permission for medical attention to be administered to my child by the camp staff in the event of a medical emergency. When I cannot be contacted, I hereby give my consent to have my child transported to a hospital emergency room and the hospital and medical staff have my authorization to provide any treatment, at my expense, that a physician deems necessary for the well-being of my child.


I hereby waive and release R.I.S.E Summer Camp and Hands of Hope America, Inc. and its trustees, officers, teachers, employees, counselors, volunteers, agents and assigns from and against any and all present and future claims, costs, liabilities, expenses, or judgments, including attorney’s fees and court costs, resulting from any damage, loss, personal injury or illness to my child and/or damage to my child’s property arising from or out of my child’s attendance or enrollment in, or out of my child’s participation in activities at or offered by Hands of Hope America Summer Camp.


Photos: Hands of Hope America has permission to use photographs of children for promotional purposes.


Dismissal of Camper: Hands of Hope America reserves the right to dismiss, in its sole discretion, any camper whose behavior is deemed unsatisfactory or detrimental to the best interests of R.I.S.E Summer Camp, themselves, other campers, and/or staff.


Health Inventory Form: I agree to fully and accurately complete the Health Form provided by Hands of Hope America with any and all information asked regarding my child. I understand that this is confidential information and it is in the best interest of my child to provide this information.


I HAVE READ AND FULLY UNDERSTAND ALL THE TERMS AND CONDITIONS AS EXPLAINED ON THIS PARENTAL CONSENT FORM AND GIVE PERMISSION AS OUTLINED ABOVE.

Refund and Cancellation Policy


  • Hands of Hope America reserves the right to cancel any program that does not meet enrollment requirements or due to a significant health crisis. All schedules, activities, class fees may be changed or canceled by HOHA without notice. If HOHA cancels a fee-based class or program for any reason, a credit toward another program/session will be applied.
  • All cancellations and refunds are subject to a nonrefundable deposit per account holder. No refunds will be issued for any reason, including but not limited to, cancellation by the account holder, change in personal circumstances, or medical reasons.
  • In the event a program/class is canceled due to weather, unexpected emergency, facility improvement, or community-related concerns, HOHA will issue a prorated program credit for each week missed minus the non refundable deposit.
  • Not attending a program/class does not entitle an account holder to a refund. When an account holder claims a spot for a program/class, the account holder is reserving the space, time, and staffing, whether or not he/she attends.
  • Extenuating circumstances may be reviewed and potentially approved at the discretion of HOHA. No refunds are granted for late arrivals, no shows, or who are asked to leave a program/class for behavioral issues.

Summer Camp COVID-19 Refund Policy


  • If Hands of Hope America cancels a program/class, we will be offering credit to your account for the canceled camp week(s). A credit toward another program/class will be issued. If you request to cancel your child’s registration due to COVID-19 related concerns at any time prior to the start of the camp week, you may receive a full refund, minus the nonrefundable deposit. A positive PCR test with a doctor's note must be provided for refund. Cancellations for other reasons are subject to our no refund policy. Families who do not inform HOHA of possible COVID-19 symptoms or exposures in a timely manner may be removed from camp registration without a refund.
