Case ManagementEverything starts with Case Management. We will take the time to get to know the needs of you and your family, and assess how we can best help. When we’re finished with your Case Management evaluation, you will receive a CASE NUMBER, which you will need to access the Hands of Hope America services.

Let us assess your treatment needs, developing, monitoring and evaluating treatment plans and progress. We can facilitate interdisciplinary approaches and monitor staff performance.

To schedule a virtual appointment (available Tuesday, Thursday and Friday 9am-3pm) you will need:

  • A computer, or laptop
  • Access to Wi-Fi, or ethernet

If you do not have these options, let the case manager know in advance so you can use one of our computers at the Community Center.
